Straps in Place
Pros: Even frozen it stays soft and wraps around affected limb! Stays in place with strap. I can wrap it around my ankle and walk. Better than medicine for pain. I keep 2 in the freezer at all times so if one thaws, then I have another one to use. The bag comes apart eventually and leaks the pearls. I've only used it for cold. I bought it in August 2022 and we used it intermittently. Christmas Eve through end of March everyday. It leaked by the end of March. I'm buying it again.

Great product but strap is too short for large people
This is even better than the first Therapearl back pad I bought. The pearls fill the pad fully unlike the original. It hold heat well. My suggestion is to increase the length of the elastic band. It is too short for large people. I estimate the diameter is 38 inches when not streched and 41 when streched. I would prefer two straps instead of one.

I like it overall. It also can be strapped around your knees (if you are sitting). Only drawback is I wish it stayed cold longer.

Just Bought A Second
This might not show up as Verified Purchase yet, but I just purchased a second one of these TheraPearl Back Wraps. The reason I'm purchasing another is twofold. First, I need one for the other Knee. Second, there must be a micro-hole in my original. It looks funny and doesn't appear to have as many pearls, although I haven't seen any snicking away... and the wrap still works. MY EXPERIENCE I had partial knee replacement earlier in the year. Lots of pain and swelling in the old mom-knee. I was lucky that someone loaned me one of the cooling devices; and I wish I had had one from the start. Don't skip that. I sent it back though and still need cooling, pretty much like what they give you in Physical Therapy. Even now, after walking the dogs, I still need some cooling and it's been months. I have the Back Wrap and the Knee Wrap and I prefer the Back Wrap and the reason is that it can be adjusted to cool the back of the knee as well as the sides and tops. In fact, I have found that I can use the straps to keep the wrap in place. The Knee Wrap works but is better for those that need the cooling below and above the knee. There is also no cooling over the patella where the incision is. SUMMARY The Back Wrap works for backs! I've used it hot and cold and it works fine. I'm buying another so that I can cool both knees, if need be, or extend the cooling of the one frankenknee. I find the Back Wrap to be superior for my knee usage because it cools front and back.

Good product, could be improved
A very good product, though the pictures make it seem bigger than it actually is. I use it mostly as a heat pad, and can confirm that when heated up according to the instructions, it does last for 20 minutes, as stated by the manufacturer. I should add however that the best way this works for me is by pushing all the beads into two of the three segments, and folding the third segment on top of the others to keep the beads from moving around (see pictures below, on the right, with all beads pushed form a segment to the others, on the left with the now empty segment "doubled" over the rest). This provides the best heat level for me, though of course preferences may vary. The downside is that it will heat only two thirds of its potential area. This is ok for my current problem (frozen shoulder), but it would not be good e.g. for my lower back, as using only two out of three segments would not take the whole of the back. So I think that those reviewers who complain about heat not lasting long enough do have a point, and I suspect this would be easily fixed by simply manufacturing this product with a larger number of beads. With this workarounds, it does the job for me.

great for back pain relief
Second time I have purchased this product and found great pain relief. I also use it for pulled hamstring injuries Amazon delivery driver was also a brilliant ambassador, thunder rain but still a warg cheerful greeting and smile from him.

best yet
I have had lower back issues for many years and have got relief from a variety of gel belts and packs , this one is the best yet it covers a good area providing better relief , only a minor issue the belt could be more central but I will be ordering a spare , i have only had it 24 hours and cant imagine life without it

This back pack worked very well for me until today....
....when it burst open at a seam and sent dozens of tiny gel balls all over the floor. I've had it for 4 years, and used maybe 5 or 6 times per year. Would have expected it to have lasted a bit longer hence only 3 stars

Great while the heat lasts....
Great in theory and lovely quality item however it seems to be extremely hot when it comes out the microwave, to the point where it's almost too hot to wear, and then too cold within 5 minutes. Disappointed as I was greatly looking forward to using this and I often suffer from mild back pain, however it's not completely pointless. I'm yet to try it chilled yet but the heat side of things was disappointing.
